Liability Waiver

By engaging the services of Becoming Holistics, its owner(s), or any of its agents, or by viewing or following any of the content presented by Becoming Holistics, its owner, or any of its agents, hereby agree to the following:

1. I understand that yoga, bodywork, and energy work are not a substitute for medical or mental health attention, examination, diagnosis, or treatment. I have and will consult my primary care provider prior to and regarding my participation in any session of yoga, body work, energy work, or any other offering by Becoming Holistics, its owner(s), or any of its agents. I certify that I am physically and mentally fit and I have no medical condition which would prevent my full participation in yoga, bodywork, energy work, or any other offering by Becoming Holistics, its owner(s), or any of its agents. I recognize that it is my responsibility to notify my practitioner of any pregnancy, potential pregnancy, serious ailment, or injury before every session.

2. If at any time during a session I feel discomfort or strain, I will gently release the posture and inform the practitioner. I recognize that yoga, bodywork, energy work, etc. may require some physical exertion, which may be strenuous and may cause physical injury. I am fully aware of the risks and/or hazards involved. It is important that I listen to my body and respect its limits on any given day.

3. I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in any program with Becoming Holistics, its owner(s), or any of its agents.

4. I knowingly, voluntarily, and expressly waive any claim that I may have against Becoming Holistics, its instructors and staff, and its owner(s), for any injury, death, or damages that I may sustain as a result of any offering of Becoming Holistics or its agents, including loss that may be caused by the negligence of the released parties.

5. I release and discharge Becoming Holistics, its directors, owners, staff, and practitioners from any and all liability, claim, demand or action that I may have related to the loss, theft or damage of any of my personal property while attending any of their offerings.

6. I understand that all content provided, whether in person, over the phone, online, or through any form of media, is for informational and entertainment purposes, and is not medical, religious, business, or financial advise. I will use my personal discernment about all my choices, including but not limited to what actions are right for my body and mind and how I should govern my life.  

7. I, my heirs, or legal representatives, forever release, waive, discharge and covenant negligence or other acts.

 

I have read the above release and waiver of liability and fully understand its contents. I am 18 years of age or older and voluntarily agree to the terms and conditions stated above.
